Report Overview
The Safety Management System Case study was a comprehensive analysis of the Safety Management System and safety culture at “Quest Airlines” following an engine power loss incident. Our final report showed the root causes of the incident and provided recommendations to improve the safety of the organization.
My Role
I contributed to analyzing the four safety oversight elements, which are the reactive processes, proactive processes, investigation, and analysis. I detailed the role of the Aviation Safety Action Program and contractor evaluations. I connected pilot complacency and training oversight to the root cause of the incident.
Final Outcome
The final deliverable of the Safety Management Systems case study was a case study report where the solution was found. We determined that the root cause was a procedural failure on the contractor, which was made worse by a lack of security protocols. We recommended comprehensive vendor training audits as part of the report.
Conceptual Framework
The project was grounded in the theory that a good safety culture acts as a defence against organizational accidents. We dissected the airline’s operations into three subcultures. Those are Informed Culture, Just Culture, and Reporting & Learning Culture.

Our Group’s Collaboration
As a group, we developed this report collaboratively. We shared a document for real-time compiling and editing. Throughout the project’s timeline, each member peer-reviewed each other’s sections. We all made sure the final product fitted a cohesive narrative.
